If you are interested in coming to New Moon Lodge or referring someone to our program, the process is as follows:
Fill out the contact form below and submit. Then download the forms located at the bottom of this page, Including our Referral Packet, Medical Clearance, Consent for Release of Information, Personal Belongings List, Client Quarantine Schedule and Covid Information. Complete the Referral Packet and have a physician complete the Medical Clearance. Email the Referral Packet, Medical Clearance, a copy of the client’s CIB and insurance, and any appropriate Consents for Release of Information to email@example.com.
After receiving the required documents, a phone interview will be scheduled to determine if our program is a good fit for the client’s needs. You should hear from us within 3 business days regarding acceptance into the program.
If you are waiting on an appointment to get the Medical Clearance forms completed, you may send the referral packet and copy of the client’s CIB and insurance so that we can schedule a phone interview earlier; but we will not be able to consider accepting the client into the program until the Medical Clearance form is received. If accepted into the program, we will call and schedule an intake date.
